1. Proof of Citizenship (Any ONE of the following)

Birth certificate

clip_image001[2] Either parents’ passport/proof of citizenship AND Parent’s Ration Card with applicant’s name in it.
clip_image001[3] Either parent’s passport/proof of citizenship AND applicant’s birth certificate (for applicants born out of India)
clip_image001[4] Sworn affidavit stating place and date of birth AND extract from Electoral Roll stating name, father/spouse name and age.
clip_image001[5] Sworn affidavit stating place and date of birth AND school leaving certificate for 10th or 12th standard issued by ICSE, CBSE or any State Education Board.

2. Proof of Residential Address (Any TWO of the following)

3. Additional documents required ONLY in case of change of name (Any ONE of the following)

Where Proof of residential address is in the name of spouse/parent/legal guardian,Plesase Provide :

    1. Hi Kenneth koirala,

      It depends on the place you are living in right now. If you a from Nepal and currently residing there, then you may want to contact your local Police Station for such a verification proof.

      The Police verification process mentioned is only applicable for people who have applied for a Passport. The verification request is sent by Passport authority to police department.

    IF YOU ARE RENTED THEN GO TO THE RESPECTIVE POLICE STATION AND ASK THEM FOR POLICE VERIFICATION. YOU ALSO CAN TELL YOUR LAND LORD FOR THEM TO GET DONE YOUR POLICE VERIFICATION.

    1. Irrespective to the need of passport, please have your DOB Certificate fixed/updated. You can contact your municipality for re-issuing the new DOB certificate with proper details. You’ll need DOB Certificate to be correct for even various other legal checks/verification.

      In case, if you are in hurry for passport, you can please add your other certificates that prove your DOB along with surname, something like SSC, Community and Nativity cert., etc.,

    I submitted my passport application at PSK Malad (Mumbai) on 24/3/15. As a senior Citizen (reissue of Passport) I thought things will be little easy. I was sent to special counter for checking of Documents (For Seniors and Infants). The line was much less compared to normal category. I had taken all documents and copies. They told me that photocopies have to be in A4 size. So I had to copy my old passport (first two and last two pages) again over there.
    Then I was given the token no. and waited for my turn. In Zone A (Managed by TCS) I was photographed and finger printed and was told to wait for my turn in Zone B. When my turn came at zone B my passport and documents were rechecked (this time by Passport Office staff). I was told that since my old passport was issued abroad, there will be pre verification by Local Police.
    I was told to wait for Zone C. At Zone C my papers were again checked (this time by Passport Officer, Govt. of India). He told me my father’s name is only mentioned with initials and since their computer system does not accept initials I should provide full name. I was sent back to Zone A for correction. I got my father’s name in full and showed them my PAN card as proof of full name. Then again to Zone B and then Zone C. Finally the Officer at Zone C accepted my file and told me that there will be Police Verification eventhough the address has not changed.
    On 25/3/15 I got SMS from Mumbai Police that I should visit Local Police Station for verification within next 3 days.
    On 27/3/15 I went to Local Police Station where I was told that a Police Constable will visit your house first. Only after that you should visit local Police station with documents.
    Luckily Police visited my residence on 27/3/15 and gave me a small chit informing me what documents I should bring for verfication within next 4 days The list read as below:
    2 copies of
    1) Date and Place of birth
    2) Photo ID
    3) Society Letter
    4) Electric bill Current and 12 month back
    5) Ration Card
    6) bank Account.
    7) Other documents what you submitted at Passport Office.
    8) 3 Passport Size Photographs

    I made 2 sets of above documents and went to Local Police Station (this time in A4 size)

    The Police constable told me only one set and 2 photos are required. He checked the papers and my Old Passport. Within 5 minutes all formalities were completed. He gave me his completed check sheets ( 2 pages) and told me to get photocopy of the same. I went down to Zerox shop and got the photocopies and gave it to him. He said he will send all clear report within a week.

    I am giving this report for the sake of others to avoid hassles.
